Request for Permission to Travel: If a student has plans to travel with his/her host family for trips longer than a week-end, or without his/her host family, this form needs to be filed.

Criminal Background Check Form: All host family members aged 18 and up must complete this form so that we can do the required background check. All Community Coordinators and Regional Directors must also complete this form.
Host Family Application: If you are interested in being a host family, download and fill in this form.
Community Coordinator Application: If you would like to work for iE-USA as a community coordinator, use this application form.
iE Student Insurance Claim Form: For current iE Exchange students who have the AVI Health, Accident and Liability Insurance. You can download this form if you need to make a claim for medical bills, accidents or liability.
